NURSING HOME

The Election Commission will obtain a list of residents from the nursing home before each election.  We will check that list to see who is a registered voter, and will go to the facility before the election to allow them to vote.  Persons may vote at the office during the early voting period if they wish.

To be eligible for placement on the register, a voter shall file a statement by the person’s licensed physician with the county Election Commission stating, under the penalty of perjury, that in the physician’s professional medical judgment, the patient (voter) is medically unable to appear at the polling place to vote and is medically unable to go to the commission office for early voting.  The statement shall be filed with the election commission office not less than (5) days before the election.  You may print a Physician’s Statement form below, or pick one up at the Election Commission office.  Once this form is filed, Individuals on the permanent absentee register are automatically mailed an application to vote by mail before each election.  Voter may not go to the polls on election day, or vote early as long as on Permanent Absentee.
